Job summary: Arkansas Support Network is seeking an experienced Supports Coordinator to join our Community Living Services Department. The Supports Coordinator is responsible for managing and coordinating services for individuals receiving support from the agency, with a particular focus on integrating and optimizing the use of enabling technologies to enhance independence and quality of life. The job involves ensuring that individuals receive appropriate support, assistance, and services through the agency.

What You’ll Do
- Coordinate and oversee the implementation of Charting the Life Course framed support plans and provide direct supervision to the DSP staff supporting the individual served.
- Work with agency teams to ensure compliance with relevant regulations and guidelines governing agency services.
- Implement individualized service plans based on the unique needs and preferences of each individual.
- Collaborate with individuals, their families, and support teams to execute comprehensive support plans created using the Charting the Life Course framework.
- Identifies and works directly with ‘internal champions’ to facilitate and support the person-centered use of enabling technology support.
- Monitor resources efficiently to meet the needs outlined in the individual's service plans, including the allocation and management of enabling technology resources.
- Work collaboratively assisting the Personnel team with monitoring SL scheduling and direct in person support and remote support.
- Conduct monthly home visits, review technology plans, perform 60-day progress reviews, and review/approve DSP daily activity records, with a focus on the effectiveness and user satisfaction of implemented enabling technologies.
- Collaborate with various stakeholders, including healthcare providers, technology team, technology coordinator and community organizations.
- Establish and maintain effective communication with individuals, families, and support teams as the liaison and point of contact.
- Maintain accurate and up-to-date documentation of services provided, including detailed records of enabling technology implementations, usage, and outcomes.
- Work collaboratively and cooperatively with departments and teams to facilitate agency compliance responsibilities and areas of improvement.
- Establishing a strong and supportive relationship with the individuals and families receiving support. Serve as the primary point of contact and resource coordinating all aspects of support including crisis and emergency situation management for the person receiving services.
- Coordinate with the technology team to design and implement customized technology supports that align with individuals' goals and support plans.
- Collaborate with the technology team to troubleshoot issues and ensure optimal functioning of enabling technologies in individuals' daily lives.
- Facilitate the collaboration between traditional in-person support to technology-enhanced support models where appropriate.
- Other duties and responsibilities as assigned may vary depending on the individual and agency’s needs.
What You’ll Need
- High School Diploma.
- Minimum 3+ verifiable years of supervisory experience.
- Strong understanding of the agency's programs.
- Ability to develop people focused relationships.
